Business Registration Checklist
For individuals that are in process of completing an 'Independent Services Provider' agreement with one of our campuses for items such as guest lecturers and similar honorarium payments, please see the following instructions:
The Business Registration checklist is intended to highlight some of the critical information you will
need to provide during the registration process. Having this information ready
will assist you in completing the registration process quickly. The checklist is
not meant to be comprehensive of all information required of you. Much of the
information you will be required or asked to provide you should know or have
readily available.

Be prepared to provide your business’’s EIN/TIN number or if not applicable your social security number.

Know the manner in which your business was formed (i.e. incorporated, partnership, sole proprietor, etc.) and in which state.

Be prepared to provide a valid email address which will be used to receive your business’ copy of any purchase order issued by NSHE.

Be prepared to provide a completed W-9 or W-8 form. You can download tax forms from http://www.irs.gov. The form must be completely filled out, signed and accessible in electronic format so that you may upload it during registration. Your business’ registration will not be processed without a properly completed and submitted W-9 or W-8 form.

Should your business have any certifications such as WBE, MBE, SBA, VBE or any others, have an electronic copy of the certification form for upload.

Have ready any business licenses (municipal, county, state, contractor’s, federal, etc.) your business has acquired. You will need the expiration date and the license number. We also request you have an electronic copy of the license to upload.

Be prepared to provide detail regarding any types of insurance your business carries including General Liability, Workers Compensation, and Automobile. If other types apply, please include them as well. You will need the expiration date of each type and an electronic copy of the certificate(s) to upload.

below. Please ensure that you have these documents available in electronic form so that
you can easily attach them during the registration process.
